Welcome, and thank you for supporting the events put on by MAMA! To follow is our online form that must be completed to be able to have a booth or truck at the event.
- Complete the Vendor Application form in its entirety with the appropriate fees. Fees must be paid by Monday November 16th, 2024 deadline. NO LATE APPLICATIONS WILL BE ACCEPTED!!!!!
- General liability insurance for CiM, is strongly encouraged. If you have certificate please provided with the following names listed as “additional insured”: Middletown Area Merchants Association and County of Lake. This protects you and the additional insured in case of injury in or near your booth. If you do not already have a policy, a one day policy can be purchased at your local insurance company.
- Complete and digitally sign the Hold Harmless agreement.
- Food vendors must have a permit from the Health Department, that form is also attached to this packet. It must be completed and turned in to the Health Department Vendor Permit Application (PDF) Vendor Permit Application (Spanish) (PDF) , Approved Health Permit must be turned in during event check in.
- Product vendors must have a California Seller’s Permit as per state law, this can be obtained by visiting their website https://onlineservices.cdtfa.ca.gov/
- If you require electrical for your booth or truck, you must provide your own power source such as generator. Each vendor will be responsible for supplying their own canopies, tables, chairs, signage, etc.
- Payment for booth; Credit/Debit at the end of this form. No other payment method with be accepted.
- EVENT WILL GO ON “RAIN OR SHINE”, BOOTH FEES ARE NOT REFUNDABLE UNLESS EVENT IS CANCELED BY HOST.
- Please do not remove your booth during the event for any reason and stay until the end of the event.
- Clean-up: Vendors are responsible for cleaning all trash and waste including sweeping up any debris within and around the allotted space. Please pack out your own trash, failure to do so will incur a $20.00 fee
