Vendor Sign-Up Instructions
Event Date: Saturday, December 13, 2025
Event Time: 3pm-8pm
Welcome, and thank you for supporting the events put on by the Middletown Area Merchants Association (MAMA)! Please read carefully and complete all requirements to participate as a vendor or food truck.
✅ How to Register

MAMA Members may sign up at no cost — please sign in first to access the member option.

All other vendors must:

Complete the Event Registration Form (Business & Vendor Application) in full.

Pay Booth Fees by Monday, December 1, 2025.

Applications are void if fees are not received by the deadline.

Payment accepted by Credit/Debit at the end of this form only (no cash or checks).

📑 Required Documents

Insurance (strongly encouraged):
If you have liability insurance, please upload a certificate naming both:-
Middletown Area Merchants Association

County of Lake
as additional insured.
➝ If you don’t have coverage, ask your insurance agent about a one-day event policy.

Hold Harmless Agreement:
Must be digitally signed with this application. -
Food Vendors:

Must obtain a Health Department Permit.

The Health Dept. application must be submitted to the County of Lake by Friday December 5th.
- Important: Even if you are giving away food or drinks (cookies, cocoa, coffee, cider, etc.), you are still required to have a Health Department permit.

Product Vendors:

Must hold a valid California Seller’s Permit (per state law).

⚡ Booth & Equipment Rules

Power: Vendors must bring their own power (generators, etc.).

Setup: Bring your own canopies, tables, chairs, signage, and displays.

Event Policy:
-
Event will be held Rain or Shine.
-
Booth fees are non-refundable unless the event is canceled by host.
-
Vendors must stay until the close of event (no early takedowns).

🧹 Clean-Up
Vendors are responsible for removing all trash, waste, and debris from their booth space.

Pack out your own trash.
-
Failure to clean up will result in a $20 fee.
